I won't go into much detail, but a hardware modification is required that allows one to copy cached map files from the Xbox's hard drive to a PC, where they are modified with special software and then copied back to the Xbox.
This is mostly just a novelty now that Halo is out for the Mac and PC, and the official Halo Editing Kit (HEK) will (hopefully) be released soon.
